| 内容記述 | 本研究では, 早産児の痛みのアセスメントがベッドサイドで容易にできるよう, 上部顔面表情運動の定量に基づいたフェース・スケールを開発することを目的とした. 対象者は, 在胎32週未満で出生した早産児10名で, 在胎週数は29.6±1.7週, 出生体重は1,262±238, データ収集時の修正齢は33.1±2.4週, 日齢は23.5±14.6日, 体重は1342±314gであった. 顔面表情, 処置場面, 心拍・呼吸モニターをビデオカメラで撮影・録画し, 顔色は肉眼的に観察し, 顔色の変化や顔面蒼白の出現を記録した. 痛みを伴う処置は栄養チューブ(経口・経鼻)の抜去と挿入, 採血(足底穿刺と手背穿刺の穿刺と搾りとし, 計29場面, 60データを分析対象とした, 上部顔面表情運動の定量は, ビデオ運動解析システム(Dynas 3D/G)を用い, 顔面に貼付した4点(左右の眼窩上縁・その中点・鼻根)から成る面積の最小値(最大顔面表情)を求め, 次に, 安静時をベースとした顔面積変化率をコンピュータで解析した. 上部顔面表情の分類は, 最大顔面表情について, 表情筋の動きによって形成される皺を記述し, 皺形成の共通性から分類した. 皺形成によって分類された上部顔面表情について, 最大顔面積変化率, 対象者の背景との関連を一元配置分散分析によって求め(有意水準5%), 顔面表情をレベル化した. さらに, 先行研究から, 本研究では出現しなかった反応を加え, 5つのレベルのフェイス・スケールを作成することができた. 今後は, 本スケールの信頼生, 妥当性を検証することが課題である. | |||||||||||||

| 内容記述 | The purpose of this study was to develop a face scale based on quantitative analysis of upper facial motion by three dimension behavioral analysis in order to facilitate assessment of pain of preterm infants in clinical settings. The subjects of this study were ten preterm infants who met the inclusion criteria of being less than 32 weeks gestation at birth, having no neurologic abnormalities, and whose parents gave consent. Data were collected at 29 sessions for routine painful procedures (heel stick, venipuncture and exchange of naso/oro gastric tube), and 61 data sets (puncture and squeeze, extubation and intubation) were analyzed. Upper facial motions were videotaped and analyzed quantatively with computer. The smallest area of shape formed with four points (right and left upper orbital, midpoint between them, and nasion) was calculated for each procedure. The rate of change from the quiet phase to the painful stimulation phase was calculated. Upper facial expressions were classified into four levels by the lines made on upper face. Each facial expression was analyzed for the relation of change rate and the back ground of the preterm infants with one-way ANOVA. The face scale for pain assessment of preterm infants with five levels was developed from those of the results and earlier literature. The future issue is verification of reliability and validity of this scale. | |||||||||||||
