本調査は、看護管理者の認定看護師(新生児集中ケア)の受け入れに関する認識を明らかにし、臨床への活用の可能性を探ることを目的とした。ハイリスク新生児の病床が独立看護単位である16施設の看護部長または総婦長の回答を記述的に分析した結果、次の知見を得た。①全員が認定看護師(新生児集中ケア)は必要と回答し、その70%が「是非必要」および「是非研修に派遣し、できるだけ早く採用したい」と回答した。②採用を考えている場合、50%が「研修は出張扱い」で「給与は全額保証」、「交通費は全額保証」と回答した。③全員が「認定看護師に役割と責任の権限を与える」と回答した。これらの結果は、1996年のスタッフ・主任・婦長を対象にした調査結果と同様であった。看護管理者の認定看護師(新生児集中ケア)に対する認識は肯定的かつ積極的なものであり、臨床での活用の可能性は大きいものと考えられる。より円滑な活用には、認定看護(新生児集中ケア)の役割、責任と権限が具体に明示、理解される必要がある。また、認定看護師制度を複雑化しないために、他の既存の研修・認定制度との調整をする必要がある。 |

The purpose of this study was to identify the nursing administrators' recognition about the acceptance of the certified (neonatal intensive care) expertized nurse and to find the possibility of make use of the certified expertized nurse clinically. The questionnaires were sent to the nursing administrators of 16 hospitals which have the neonatal intensive care unit As a result. 1) everyone answered that the certified expertized nurses are necessary, and 70% of them answered "certainly necessary" and "certified expertized nurses will be dispatched to the in-service training from time to time, and desired to be employed as soon as possible." ; 2) If the employment is under the consideration, 50% answered that "the in-service training will be treated as an official trip", "the payment will be fully insured", and "the traffic expenses are fully paid." ; 3) Everyone answered that "the role and the responsibility will be given to the certified expertized nurses." These results were as same as those of the study undertook toward the staff nurses and nurse managers in 1996. The recognition for the certified expertized nurse by the nursing administrators is positive and also active, and the possibility for the make use of the certified expertized nurse clinically will be big. For the smooth use, the role, the responsibility, and the authority should be clearly stated and be understood. And also, in order not to confuse the certified expertized nursing system, it should be distinguished from other in-service triaging and the recognized systems. |
