| 内容記述 | 21世紀に入り、小児医療においては、施設内医療から在宅医療へという医療システムの変化に伴い、子どもの入院期間を短縮させる方法が模索されている。長期入院患児には、呼吸器から離脱できない子ども達が含まれている。1950年代から1960年代にかけて急速に呼吸器の開発が進み、今や携帯用の呼吸器はラップトップヘと進化した。そして、呼吸器へのイメージは、「生命維持装置」から子ども達の世界を限りなく広げる「魔法の箱」となり、呼吸器は日常生活用具として考えられるようになった。そこで、呼吸器から離脱できない長期入院中の子どもの親は、子どもの今後における生活場所の選択を迫られる横会が増えた。しかし、子どもが呼吸器を装着したまま地域で生活するには解決すべき課題が多い。その上、病気の経過が明らかではなく、親は将来の見通しが得られない不確実な状況の中で在宅を決断しなければならない。看護者の役割は、子どもや親・家族の権利を尊重し、自己決定を支援することである。そこで、ここでは親の視点に立ち、呼吸器を装着した子どもの親を対象に行った2つの調査結果を基に、在宅への移行にまつわる親の認識を述べ、親の意思決定を支援するための看護援助を考えたい。親は子どもの発病から在宅への移行を決断する過程において、医療者との関わりから子どもの「命」と「育つ」ことへの医療者との認識のずれを経験していた。そして、親は子どもの存在に対する意味づけを変化させながら、在宅を決断していた。そのような在宅を決断するに至るまでの過程における親の認識の変容には、子どもが置かれている状況への親の不確実な思いが大きく影響していた... | |||||||||

| 内容記述 | With the general shift from hospital care to home medical care for paediatric patients in the 21st century, several ways to reduce the duration of hospitalization of children have been sought. Most children hospitalized for a long time include those who must use a respirator at all times. Due to rapid progress in the development of respiration apparatus from the 1950s to 1960s, portable laptop type respirators are available today. The impression of a respirator has changed from a "life-sustaining apparatus" to a "magic box" that potentially opens up the world to the children who use it. Currently, respirators are regarded as a tool commonly used in daily life. As a result, the parents of a child who uses a respirator and is hospitalized for a long time often face the difficult choice of determining where their child should live. However, there are still many problems to be solved in order for children to stay at home in their local community using a respirator. Furthermore. parents must decide whether they should look after their child at home or keep him/her hospitalized without fully understanding the prognosis of their child' S illness and without knowing what the future may bring. One of the missions of nursing staff at the hospital is to help in this process of decision-making, while respecting the rights of the children and their parents and family members.
