| 内容記述 | 虚血性心疾患と脂質代謝異常との関連についてFramingham Studyを始め多くの報告がある。アポリポ蛋白遺伝子・脂質関連酵素遺伝子解析と脂質代謝・冠動脈疾患との関連についても多くの報告があるが,日本人を対象とした報告,特に冠動脈疾患との関連についての知見は乏しい。遺伝子解析の方法として疾患群と正常群とを比較するAssociation studyと冠動脈病変の重症度の評価により,日本人虚血性心疾患患者におけるアポリポ蛋白多型と血清脂質関連項目・冠動脈病変との関連を明らかにすることを目的とし以下の検討を行った。 当科にて冠動脈疾患を疑い冠動脈造影検査を施行した80症例(M:F=48:32,平均年齢57.1歳)を対象とし,AHA Comitte Reportに基づき冠動脈狭窄度により3群に分類し,また冠動脈重症度の2つの指標を算出し検討に用いた。血清脂質関連項目として総コレステロール(以下TC),中性脂肪(以下TG),高比重リボ蛋白コレステロール(以下HDL-C),アポリポ蛋白AI,AII,B,CII,CIII,E(以下Apo AI,apo AII,apo B,apo CII,apo CIII,apo E), Lipoprotein(a)(以下Lp(a)),Remnant like particle-cholesterol(以下RLP-C)を定量した。アポリポ蛋白遺伝子多型はapo AI/CIII/AIV遺伝子多型[Msp I,Sac I restriction fragment length polymorphisnl(以下RFLP)],apo B遺伝子多型[Xba I RFLP,signal peptide(以下SP)多型,variable number of tandemly repeated short DNA sequences(以下VNTR)多型],apo E遺伝子多型の6つについてpolymerase chain reaction(以下PCR)法を用い同定した。アポリポ蛋白遺伝子多型と血正清脂質関連項目・冠動脈病変との関連を統計解析し以下の結果を得た。 1) 血清LDL-C,LDL-C/HDL-C比,ApoB/ApoAI比は,冠動脈狭窄を有する患者群で高値であり,また冠動脈病変重症度と有意な相関を認めた。 2) Apo AI/CIII/AIV遺伝子多型のうちSac I RFLPのS2 alleleはRLP-Cの有意な高値を示し,TG異化障害との関連が示唆された。 3) 女性のsubgroupにおいて,Apo B遺伝子多型と血清apo B値,apo B/apo AI比との関連が認められた。 4) Apo E遺伝子多型のE4カテゴリー(ε3/4,ε4/4)は,E3カテゴリー(ε3/3)に比し血清LDL-C値,LDL-C/HDL-C比,ApoB/ApoAI比の高値を示し,apo E遺伝fのコレステロール転送系への遺伝的関与が確認された。 5) 女性のsubgroup特に55才以上の閉経期女性においてApo B遺伝子SP多型と冠動脈病変重症度との関連が認められた。 以上よりアポリポ蛋白遺伝子多型は日本人において血清脂質・冠動脈疾患を規定する因子であることが示唆された。特にApoB遺伝子SP多型は閉経期女性の血清apo B値とそれに起因する冠動脈疾患において重要な役割をなすことが推論された。 | |||||||||

| 内容記述 | To assess the impact of apolipoprotein gene polymorphisms on serum lipid traits and severity of coronary atherosclerosis in Japanese populations, apolipoprotein gene polymorphisms were investigated at the following loci; apo AI/CIII/AIV (two restriction site polymorphisms - Msp I and Sac I), apo B (Xba I polymorphism, an insertion/ deletion [I/D] polymorphism in the signal peptide [SP] region, and variable number of tandem repeat [VNTR] polymorphism of the 3' hypervariable region) and apo E (ε2, ε3 and ε4) in 80 Japanese subjects (48 men and 32 women, mean age 57.1 years old) undergoing coronary angiography. Severity of coronary atherosclerosis was found to be significantly associated with LDL-C, LDL-C/HDL-C and apo B/apo AI levels. There were significant association of serum lipid traits with genetic polymorphisms; high serum RLP-C level in S 2 allele of Sac I restriction fragment length polymorphism, high levels of apo B and apo B/apo Al in I/I genotype of the apo B SP polymorphism in a female subgroup, high levels of LDL-C, LDL-C/HDL-C and apo B/apo AI in E4 category (ε3/4 and ε4/4). UI genotype of the apo B SP polymorphism revealed a significant association with severity of atherosclerosis in a female subgroup. These findings estimated that I/D SP polymorphism at the apo B gene locus is associated with the lipid traits, which may be the postmenopausal hypercholesterolemia, as well as the subsequent development of atherosclerosis in the female Japanese populations. | |||||||||
